At Ultra Performance Physical Therapy, we understand that you may have questions about our services, appointment scheduling, and what to expect during your visit. To provide you with the information you need, we have compiled a list of frequently asked questions. If you have additional inquiries, feel free to reach out to us.

  • Do you accept insurance?
    We are contracted with Blue Cross Blue Shield and have self pay options as well as packages available to provide you quality and comprehensive care. If you have a different insurance provider we can provide a super bill with diagnosis and CPT codes for possible reimbursement.
  • Can I still schedule even if I don't have Blue Cross Blue Shield?
    YES! You can schedule an appointment under our self pay option and we can provide a detailed super bill for each session with CPT and diagnosis codes that you can submit toward your out of network benefits.
  • Why don't you accept my insurance?
    We believe in working for and with the patient and not having insurance plans dictate your care. Most insurance plans do not support the 1:1 care model and it forces clinicians to see multiple patients at one time. At Ultra Performance Physical Therapy, every patient is treated by a Doctor of Physical Therapy for the entirety of their session and is never passed off to a technician or assistive personnel. Each patient is more than a diagnosis and you are not confined to only being treated for one specific area at a time. The 1:1 care model helps patients feel better faster and therefore you do not need to attend therapy as frequently and/or need as many visits.
